Bruxism is one of the most underdiagnosed conditions in dentistry — primarily because most people do it in their sleep and have no idea it's happening. By the time I spot the wear patterns on a patient's teeth, the damage has often been accumulating for years. The good news is that it's easy to protect against once identified. What Is Bruxism?
Bruxism is the involuntary clenching, grinding, or gnashing of teeth — most commonly during sleep (sleep bruxism) but also during waking hours under stress (awake bruxism). It affects an estimated 8–10% of adults and is more common in people who:
- Experience high stress or anxiety
- Have sleep apnea or other sleep disorders
- Use caffeine heavily (especially late in the day)
- Use tobacco or alcohol
- Take certain antidepressants (SSRIs)
- Have a misaligned bite (the teeth don't meet evenly)

- Morning jaw soreness or tightness

The Damage Bruxism Does Over Time
This is what concerns me most about unaddressed bruxism. The forces generated during grinding can be 10–40 times greater than normal chewing forces. Over years:
- Enamel wears flat — enamel doesn't grow back. Once worn, the underlying dentin is exposed, causing sensitivity and further accelerated wear
- Teeth chip and crack — stress fractures develop in enamel, leading to chipping or full cracks that may require crowns
- Restorations fail early — fillings, crowns, veneers, and implant crowns are all vulnerable to the excessive force of grinding
- TMJ damage — chronic overuse of the jaw joint leads to arthritis, disc displacement, and chronic pain that is difficult to reverse
- Bone loss around implants — grinding is one of the leading causes of implant failure in otherwise healthy patients
