"Do I really need a deep cleaning?" is one of the most common questions I get after treatment recommendations. It's a fair question — the terminology is confusing, and patients are right to want to understand what they're paying for and why. Here's the honest breakdown.
Regular Cleaning (Prophylaxis) — For Healthy Gums
A standard cleaning — technically called a prophylaxis — removes plaque and tartar from the tooth surfaces above and slightly below the gumline. It's a preventive procedure for patients who have healthy gum tissue with normal pocket depths (1–3mm).
- Scaling — removing plaque and tartar from all tooth surfaces above the gumline
- Polishing — removing surface stains with prophylaxis paste
- Flossing — cleaning between teeth
- Fluoride treatment (optional) — strengthening enamel
Duration: 45–60 min · Frequency: Every 6 months · Insurance: Typically covered 100% by PPO plans · Cost without insurance: $100–$200
Deep Cleaning (Scaling & Root Planing) — For Gum Disease
A deep cleaning — called scaling and root planing (SRP) — is a treatment, not a routine procedure. It's recommended when a patient has periodontitis: gum disease that has caused pockets to deepen beyond 4mm and tartar to build up below the gumline where regular instruments can't reach.
The key difference: scaling and root planing goes below the gumline, using specialized instruments to:
- Local anesthesia administered — the area is fully numb
- Scaling — removing tartar and bacteria from below the gumline, all the way to the base of the pocket
- Root planing — smoothing the root surfaces so bacteria can't re-attach as easily
- Antibiotic therapy may be placed in deep pockets
Duration: 1–1.5 hrs per session · Sessions: Usually 2 (split by side or quadrant) · Insurance: Covered 50–80% by most PPO plans · Cost: $150–$350/quadrant
